Botox and Fillers Applications

Botox and Filler applications are non-surgical methods which are used to eliminate aging problems such as facial wrinkles and deep grooves. Botox and Fillers can be used alone, but they are more effective together.

What is Botox?

Contrary to common belief, Botox is not made of snake venom.  It is made of a spasmolytic neurotoxin called “ Clostridium Botulinismis” produced by the bacteria which is formed in anaerobic environments and responsible for food poisoning. In the 1980s, it received it’s drug ID as Botox. It is the most commonly performed aesthetic interventions in the world and the FDA and the Ministry of Health approved. Botox is specially used in the wrinkle/line treatment and sweating treatment. Our facial expressions have a significant share of reflecting our emotions and feelings. Wrinkles are mostly caused by mimicking muscle, which we use in our every emotion and expression. As a result of aging, visible lines and wrinkles are formed on the skin over the muscle. In particular, wrinkle forms are more visible in early times, in active use of mimic muscles when there is no tissue sagging yet. Applying Botox during this period, temporarily paralyzes the facial muscles, corrects wrinkles, smooth appearance and prevents the formation of new wrinkles.

How to Apply?

An effective local anesthetic cream is applied to the face before the application and the application area is anesthetized. And then Botox is administered by using smaller needles. The tip of the application needle is so thin that you feel very little pain. There might be little bruises and swelling which is barely seen and disappear completely in 1-2 days. Immediately after the procedure, patients can continue their daily activities without any problem.

Which areas is it applied?

Botox is often applied to the mimic muscles between eyes to remove fine wrinkles and frown lines between the eyebrows and eyes.  It is also effective in the forehead lines. It can be applied to lip edge and neck wrinkles, but it is not going to be as effective as other regions.

How long will it last?

The effect starts to emerge 48 hours after the injection, but fully manifests at the end of a week. 1 week is required in order to drug to be effective in the muscle. Duration of effect is 4-5 months. Therefore, it is recommended to repeat it every 6 months. In long term use, repeated injections will weaken the muscle and decrease the power of creating new wrinkles. So it will extend the time required for the reapplication of the drug. Furthermore, because there is no muscle activity, wrinkle production will stop and skin aging will slow down significantly.

Who can make the application?

Botox is effective only when it is given intramuscularly.  So it requires  a good knowledge of facial structure, muscle depth which a plastic surgeon knows very well from his previous surgery experiences.  Today this practice is also done in a lot of aesthetic centers. But you should be careful. With non-expert hands there might be dangerous risks such as eyelid impairment, abnormal muscle lifting, muscle impairment. But even the worst view will resolve spontaneously 4-6 months after Botox loses its effect.

Are there any side effects of Botox?

No side effect of Botox was detected. It has been around for more than 40 years and been used safely.

What is the filler injection?

The filler injection is one of the minimally invasive aesthetic applications. It is generally applied around age 30  for the purpose of correcting wrinkles and mid years of the 20’s for the purpose of fixing the contour.

Which fillers do we use?

We prefer temporary filling material in order to prevent allergic reactions and provide safety.  We use hyaluronic acid and calcium hydroxyapatite-containing fillings. They don’t contain animal ingredients. They contain hyaluronic acid derivatives, which are already occurring substance in the body and provide fullness by pulling water more than 100 times weight of its own. The product is degradable so it leaves the body in time and leaves no harm/side effect behind.

How long do temporary fillers last?

Depending on the person and the area we apply, they vary and usually last up to 1 year.  Effects may be permanent after regular practices.

Which area are fillers applied?

We use fillers to eliminate deep wrinkles or deep grooves that cause ghosting so often. Forehead, nose, lip groove, lip edges, the groove between the chin and between the eyebrows are the most common applications. They thicken the lips and make them appear fuller. They are applied to the areas with fine lines and deep wrinkles on the face. They eliminate neck wrinkles. They are also applied to the chest wall as well as the upper side of the neck to eliminate neck and low-cut line wrinkles.  If you want to have younger hands, the fillers may be applied to the back of the hands.

How about after the application?

After filler injection you may prevent bruises by applying an ice pack to the area. In particular, for fillers containing hyaluronic acid, which show a better effect by pulling water in, you need to drink plenty of water for the first 2 days.  You should avoid of excessive massage to the treated area.

What are the advantages of temporary fillers?

Radiofrequency techniques are around for approximately 20 years. They have extremely appeared to give very promising results in the ear, nose and throat. High-frequency radio waves are converted into energy with the help of electrodes and that energy is sent directly to the tissues. Thus, the desired effect is achieved without any need for a surgical intervention on tissues. This technique, called radiofrequency applications is generally accepted as a revolutionary innovation in a sense.

In which cases radiofrequency procedures are used?

In case of nasal congestion is caused by inferior turbinate tissue swelling, radiofrequency applications can be used only under local anesthesia. It  helps the tissue to heat and shrink. It may also be a permanent solution for patients who experience snoring and sleep apnea problems. In some cases it might be engaged into surgery as a technical assistant.

When do you start feeling the effects of radiofrequency application?

Radiofrequency methods are the methods with the least complaint by patients. A slight swelling may occur in the region a few days after the application. However, this is not a noticeable size and disappears very quickly. The net result arises after the completion of approximately 3 to 4 weeks.

Are Radiofrequency effects permanent?

Radiofrequency aims to eliminate patients’ complaints without any surgical intervention. Tissue loss occurs in the region where the application is applied. Tissue shrinks down to a smaller size and texture. However, so-called hard fibrous tissue, permanent tissue occurs. In rare cases, some patients may need to repeat the application in order to contain a permanent solution. These patients are the ones who have allergy problems and suffer from the swelling of the Concha.

Is radiofrequency application painful?

Radiofrequency application does not causre any pain or ache. Application area will be fully anesthetized prior to application and it prevents patient feeling any pain. You will not feel any pain or ache even after the effect of anesthesia.

Lower turbinates (Concha) perform important tasks such as to  filter, to heat, to moisturize the air we breathe  and to secrete. In some cases (allergy, infection, congenital) they can be larger than the size they should be and can prevent us breath freely. If we cannot lower the size by using nasal sprays or certain drugs, it is possible to reduce it permanently with radiofrequency applications. This application is very simple and can be done  under local anesthesia in clinical conditions.  There will be no pain after and the desired effects take place within 1-2 weeks.

What is the Revision Nose Surgery?

What is the difference between Primary and Revision Rhinoplasty?

Revision Rhinoplasty is a more difficult operation which requires a lot of knowledge, experience and planning. If the rhinoplasty surgery had not met patient’s expectations, a revision rhinoplasty may come into question. Would Revision Rhinoplasty be more difficult for me?

Revision Rhinoplasty is not more difficult for the patient, but the surgeon. In case of lack of nasal cartilage tissue, ear or rib cartilages would be used and there would be a second wound for you other than your nose.

It does not matter how bad a nose looks, If the nose has no previous operation, it is always easier to shape it. The surgeon can clearly see the inherent anatomical structure. However, with a nose which had gone through a surgery before, the surgeon might face with more challenges. Changes in anatomical shape and inadequate nasal cartilage can be counted as some of the challenging factors of a Revision Rhinoplasty Surgery. Even if your previous surgery has been just a Deviation Surgery -is only associated with the inside of the nose- it might bring along some difficulties for a future aesthetic nose surgery.

My initial surgery could not meet my expectations. Can I achieve my desired results with a Revision Rhinoplasty?

First, we do a proper facial analysis and a nose design, just like we do for our initial nose surgery patients. However, unlike the first operation, the current conditions of the nose are more important for a revision nose surgery in terms of results and expectations. Surgical skill plays a crucial role in the quality of a revision rhinoplasty outcome. So you have to make sure you select a good and experienced surgeon for your revision rhinoplasty surgery. Let me remind you something, the healing process may last longer than your first operation. With an appropriate surgical technique and a successful surgeon selection, there is always a chance for a better result, even on your second, third or fourth operation. You just have to clearly tell your doctor your expectations and concerns.

Can they replace my lost tissue?

Tissue loss which occurred after the first rhinoplasty can be repaired with the cartilage we get from your own ear or rib. No deformation occurs in the ear after the operation. When using cartilage from the patient’s own rib cage, the scar is successfully hidden under the breast folds.

How is the healing process after revision surgery?

The healing process may take a little longer than your first surgery. Revision Rhinoplasty can be considerably challenging because of the fact that the nasal skin may be quite adherent to the residual cartilage. There may not be sufficient cartilage left in the septum for grafting purposes which may lead to the need for ear or rib cartilage. The healing process is quite different and longer as now there is a further layer of scar forming on top of an older layer of scar.

Open Rhinoplasty technique or Closed Rhinoplasty technique?

Open and Closed Rhinoplasty Technique simply refer to two possible surgical approaches of Rhinoplasty surgery. With Open Technique nasal cartilage and bone structures can be configured in more detail. It allows the entire skin over the nasal cartilage and bone to be lifted up to find out what’s underneath. Especially in patients who have problems in the tip of their nose, this technique is more suitable. In Closed Rhinoplasty technique, all of the incisions are enclosed inside the nose. All of the work is achieved through the nostrils from the inside. Because of that there is no visible external scar. This technique is preferred if there is no genuine nose deformity. The doctors’ opinions conflict over these two methods. However, I think that it is a more accurate approach to make this decision in accordance with the needs of my patients.
