Cosmetic Nose Surgery

Rhinoplasty

Cosmetic Nose Surgery Aesthetic, as a concept that has been around for years, started to come up frequently in recent years and became an important issue in the lives of both women and men. According to the researches Aesthetic Nasal Surgery is one of the most common plastic surgery.

Our attire, our posture, our facial expressions and the shape of our face can be regarded as messages in terms of communication. Unfortunately, most of the people are perceived as cold, frustrated or distant when it comes to the first impression. No matter how friendly you are in reality, people who you have just met would judge you in a second and they do that by having a little look at your appearance. It is unfortunate, but true.  Although aesthetic surgery of the nose is simply perceived as a nose reduction surgery by Turkish people, it is way more than that. Even with a small amount of touch aesthetic nose surgery redefines the character of the face and helps creating positive effects on one’s facial expression.

If you think your nose does not fit your face line or has a deformity due to genetic causes or a previous trauma, it is possible to get rid of this annoyance by a Rhinoplasty surgery. Facial plastic surgeries can be performed after completion of the facial development. This limit is roughly 16-17 years of ages for females and 17-18 years of ages for males.

Integrity is Important for Natural Beauty

Besides all these; asymmetric facial disorders and the disproportion of nose, forehead, chin & cheek are also important details that may affect the outcome after surgery. According to your face analysis, your doctor can offer you additional applications for the solution of a problem, he foresees and these applications may be the primary factors for the success of your nose surgery. For example, if your forehead is too behind or if you have a weak chin compared to your nose, only a nose surgery cannot provide you the best result. Furthermore, you may end up with an unbalanced bad nose and disturbed facial beauty. This will not satisfy the expectations of the patients who want a natural look for sure. So that your doctor’s face analysis and planning are essentials before surgery. It is advisable to discuss all of your expectations in detail and let your doctor guide you in order to get positive results. Remember, natural harmony can only occur when your face parts are in harmony with each other and it is harmony but nothing can provide a beautiful nose.

The negative effects and unhappy expression on your face caused by your nose deformity will disappear after the surgery and people around you will feel the positive impact immediately.Positive reactions from people around us are extremely useful in increasing our confidence.

Beyond aesthetic considerations, it is seen that people who experience trouble breathing through the nose to increase their quality of sleep & life after surgery and they feel more energetic. Remember, a healthy, comfortable breathing and beautiful nose, which suits your face the most will boost your confidence and you will be better and happier with yourself. Cosmetic Nose Surgery

Rhinoplasty: Answers to your questions

1. Is Rhinoplasty surgery a painful operation?

Cosmetic Nose Surgery You will be under the influence of anesthesia during your surgery and you will not feel any pain during the operation.  All patients are advised to take pain relief medication for the first few days after surgery. Generally, you won’t feel like you need to take painkillers but some may want to use a harder painkiller anyways. The most common complaint I’ve gotten is the difficulty of breathing through the nose for the night after surgery. The next day you will begin to breathe easier after we take off the intranasal dressing which we placed in your nose the day before. Most of our patients state that they have not faced any problems during the week after surgery.

2. Are there any complications or risks of Rhinoplasty Surgery?

There is a certain amount of risk and potential complications in any surgical operation. The surgeon’s task is to anticipate them and to point them out to you. I usually inform my patients against the risk of infection and bleeding. Although excessive bleeding after rhinoplasty surgery is also very rare, my mission is to inform my patients of this possibility. If such a situation occurs, it will do within a few hours immediately after the operation. In this case, patient should not worry and let the doctor know instantly.

3. Can I combine my Rhinoplasty surgery with other facial surgeries?

To answer briefly, yes. Most rhinoplasty patients combine their rhinoplasty with other facial surgery, even with body operations. For example, it is fairly common in patients to get their eyelid surgery or breast augmentation with their nose surgery. In such situations, it is strongly recommended that you discuss the details with your doctor.

4. Is it proper if I tell my doctor that I want to talk to her/his former patients who had a nose surgery before?

Of course. We support our patients to contact our former patients who had undergone similar operations on their own. The reality is, the more information is always better when it comes to patient. This is surely an understandable request. Cosmetic Nose Surgery

5. Does my health insurance cover my rhinoplasty surgery?

In some cases, a portion of the amount of your Rhinoplasty operation may be covered by your health insurance. This might occur in some cases such as a warped nose pole, the nasal valve surgery, or if the operation is related to the patient’s previous injury or damage. In such cases, breathing related part of the restructuring of the nose can be covered by the insurance. If the operation is  considered due to purely aesthetic reasons not insurance, but the patient is fully responsible for the cost of the surgery. This should be discussed at the initial examination in terms of knowing the portion of insurance coverage.

6. Can my doctor fix the shape of my nose aesthetically and treat my breathing problem at the same time?

Yes, this is a fairly common condition. Both your breathing problem and your cosmetic dissatisfactions can be solved at the same time. Your nose will be reformed in accordance with your facial features. This would count as a plastic surgery and your insurance will not cover it. The only thing in the medical context covered by insurance companies is the problem of breathing. For extra payments, the next year’s agreements or so, you still need to contact your health insurance company.

7. What are the medical conditions that might prevent a healthy recovery after a Rhinoplasty operation?

Situations which may be threatening for a successful nose operation are usually related to the internal structure of the nose and they are rare. For example, in my opinion, patients who suffer from The Wegener’s granulomatosis syndrome (WG) -affecting the nose cartilage – are not suitable for a rhinoplasty operation. The risk of complication is very high that we avoid performing a surgery. The health of the patient has been always our first consideration. Apart from this, there are very few medical conditions that could interfere with a healthful recovery.

8. How long does it typically take between the initial examination and the surgery?

It depends on the person. Patients usually set the appointment of the operation one to four months after the inspection. For out of town and international patients, time is more valuable, so they keep it shorter. However, some patients who are so unsure about the operation might want to last it up to 6 months, even a year. It really makes no difference. Because, no matter how much time passes, the doctor will remember the patient’s condition and desired results quite clearly and will always save the patient’s personalized nose design with detailed interview notes. Whether immediately after examination or years after the initial examination, I always go over my surgery plan and make sure of all the details.

9. Open Rhinoplasty technique or Closed Rhinoplasty technique?

Cosmetic Nose Surgery Open and Closed Rhinoplasty Technique simply refer to two possible surgical approaches of Rhinoplasty surgery. With Open Technique nasal cartilage and bone structures can be configured in more detail. It allows the entire skin over the nasal cartilage and bone to be lifted up to find out what’s underneath. Especially in patients who have problems with the tip of their nose, this technique is more suitable. In Closed Rhinoplasty technique, all of the incisions are enclosed inside the nose. All of the work is achieved through the nostrils from the inside. Because of that there is no visible external scar. This technique is preferred if there is no genuine nose deformity. The doctors’ opinions conflict over these two methods. However, I think that it is a more accurate approach to make this decision in accordance with the needs of my patients.Cosmetic Nose Surgery

10. Will I have scars after surgery?

Each incision in the skin leaves a trace. When people say “ Scar” we think of big, ugly stitches. However, as aesthetic nasal surgery incisions and stitches are made in a much more controlled manner, they are very difficult to be seen visually. The Open Nose Surgery does not leave a noticeable and visible trace. At Endonasal, Closed operations there will be no traces to be seen because cuts are already inside the nose. Patients who have a situation that requires an open technique, will be letting them know and informed before surgery.

11.  Are there alternatives to the aesthetic nasal surgery?

There are a number of alternative nose aesthetic operations, but they are limited.

12. Is it true that people get runny noses after surgery?

No, it is not. It may be valid for the first 1-2 weeks after the operation, but I haven’t had a patient in such complaint.

13. Does Smoking affect my recovery after rhinoplasty surgery?

Smoking affects the healing in most surgical operations. In particular, the success of the surgery can be observed by looking at the nose skin. That is why heavy smoking affects healing and postoperative surgical outcome badly. I warn my patients against smoking all the time.

14. Is it true that homeopathic medicines reduce the swelling and bruising?

There is no scientific evidence that homeopathic medicines are useful, but we support our patients who would take advantage of them. The best way of minimizing swelling and bruising is to choose a skilled and experienced surgeon who also has an effective and gentle operation style.

15. If a graft is needed when re-configuring the nose, what kind of material do you use?

In my method, the priority is the patient’s own cartilage grafts. The nasal septum is very convenient to get septal cartilage if needed during the rhinoplasty operation. However, an additional graft may be needed in some cases. In such situations non-structural parts of the ear or a part from the flexible structure of the rib can be used.

16. Will I be asleep during rhinoplasty surgery?

Yes. Rhinoplasty surgery needs to be done under general anesthesia. The surgery usually takes 1-2 hours.

17. Can I have breakfast on the morning of the operation? How soon after surgery can I eat?

The night before the rhinoplasty operation you should not eat after midnight as in all other surgical operations. Your stomach must be empty to prevent complications. About an hour, after waking up from anesthesia, you can begin to eat light meals and drink water.

18. Can I go home immediately after my surgery?

Yes. After you spend a couple hours in our custody after surgery, you have the freedom to go home with your companion. But we strongly recommend you to stay overnight in the hospital for the first night and bring a close family or friend with you who can help you with this process. Cosmetic Nose Surgery

Depending on the type of the operation, it generally takes at least 1 week.

As self-absorbing seams are used in the nose, there is no need to remove them.

22. Is it possible to be allergic to these dissolvable stitches?

It is possible, but unlikely. A mild itching is more common. In case of an allergy, stitches will be removed.

23. Will I be given cortisone to help the scar tissue?

Usually not. A very small amount can be implemented to the upper part of the tip of the nose for patients with very thick skin or excessive swelling of face. With the open rhinoplasty technique, this possibility is higher.

24. How many times will I have to go see my doctor post-surgery?

I usually see my patients on the 3rd and 7th days for dressing. For long-term follow-up, every two months until the 6th months and after that it goes like 1st year and 2nd year.

25. When will the external splint be removed?

The light metal external splint will be removed about a week after surgery.

26. When can I breathe easy?

We can breathe immediately after surgery with the help of the thin silicon layer into the nose. We recommend our patients not to blow during the week after the operation. You may gently blow your nose after one week. Rinsing the nose with salt water is also recommended.

You can start brisk walking a few days after the operation.  It would be adequate if you are away from moderate/heavy activities such as running, cycling, swimming, weight training for a month. You can always continue from where you left. Even after a month, you need to be really careful and try to minimize the possibility of trauma to your newly operated nose. Cosmetic Nose Surgery

29. Will nose continue to grow after nose surgery?

Yes. Remember, full results of a rhinoplasty are not typically seen for up to a year after surgery. After three months post rhinoplasty, approximately 80% of the swelling disappear. Within six months time, about 90% of the swelling will have dissipated, while the remainder of the swelling can slowly disappear over the next year or longer until the final, most refined result is achieved.

31. What if I’am not pleased with the results of my nose operation?

A clear and effective communication must be created during the examination for patients’ future happiness. It is extremely important that the patient has realistic expectations. The problem may usually be a swollen nose tip or swelling along the nasal dorsum swelling. What the patient does not realize is that it is probably something temporary. In such cases, the doctor shows the pictures of the healing process and tells the patient that she/he needs to be more patient. How To Decide If The Nose Fits Your Face - Dr. Abdülkadir Göksel

How to decide if the nose fits your face?

How to decide if the nose fits your face?

Before determining the design of the nose, the type of your face also is  taken into consideration as well as facial symmetry and proportions. The type of your face may vary according to your face shape, your age, your gender, your ethnicity or even your personality.

Even though your nose design is a decision which is made by you and your surgeon special for you, there are some aesthetic judgements accepted in society.

Face shape is directly linked to one of the universally recognized beauty characteristics “the symmetry”. Round, square, oval, triangular, rectangular etc. Whatever the shape of your face, it should be designed as if you divide the front horizontally  into three equal parts 1/3,  vertically 5 equal parts 1/5 and if the profile is divided into 3 equal parts coincide with the portion of 1/3.  This ratio must be divided equally both vertically and horizontally and divided equally to other sections. So the nose area should not be wider than the eye area. When you consider this calculation, you may reach a conclusion as a small and slim nose on a round face causes deterioration and does not constitute an aesthetic appearance. The nose as a focus point has a very significant impact on the symmetrical structure of the face. The new nose must be very compatible with the general shape of the face in order not to lose the characteristic features of the person.

The relation between nose shape with the face sets as a whole with the bone structure. For example, the vertical changes may cause the face looks longer or shorter. If the central part of the nose is long it leads to the face also look longer.

The importance of the nose shape when reshaping facial profile

Not only from the front side, side profile also must adapt in the face. When shaping the nose, there are some basic rates and angles which affect the profile image such as nose-forehead angle, nose-lip angle, nose- chin angle, chin-neck angle, nose-ear size ratio,forehead-nose-chin ratios. These ratios and angles must be balanced with each other. For example, if a nose looks big from profile view, the forehead and chin angle must be carefully taken into consideration in order to achieve the symmetry and desired aesthetic look. How to decide if the nose fits your face?

A substantial amount of people does not have facial symmetry and ideal facial ratios. For example,  for patients who have a bad upper face, middle face and lower face ratios, these proportionate disorders must be considered when planning their surgeries.  In cases where the lower face is too long, the nose need to be shortened carefully in order to keep the balance. Similarly, if forehead, nose and chin alignment is corrupted some additional operations to rhinoplasty might be required, such as forehead injections or aesthetic jaw surgeries.

Determination of the nasolabial angle between nose and forehead varies in women and men. For example, when nasofrontal (nose-forehead) angle is 115-130, nasolabial (nose-lip) angle is 90-120. This ratio is wider among women than men.

In conclusion before rhinoplasty, all of these ratios are calculated and then a nose is designed which fits to face the most. Patients can also be informed with designing 3D nose images.

Submucosal resection of the inferior turbinate

Other Functional Nose Surgeries Reducing tissue with radiofrequency is not a solution in cases where the size of the inferior turbinate is dependent on the thickness of the skeleton. In such cases, mucus, which coats the nasal turbines will be  preserved and its thickness on the skeleton will be reduced. Thus, the nasal turbinates without congestion and thickness, will continue to fulfill their function even with the reduced size.

Nasal Valve Surgery

We give the name nasal valve where air enters the nasal cavity. The nasal valve acts as a flow-limiter. The internal nasal valve is usually the narrowest part of the nose. Air entry cannot be detected in patients who has nasal blockage caused by narrowing or collapsed nasal valve  along with the deviation. Thus, the problem of breathing continues and the operation fails, If this problem is not cured along with deviation. Therefore, the region should be examined if there is  an angular stenosis causing obstruction.

Nasal decongestants sold in pharmacies or anti snoring nasal strips are actually expanding this area to simply breath better. However, you cannot live with nose bands forever. Permanent surgical techniques can be applied in order to widen the nasal valve. Nasal valve surgery can be performed with the deviation, with cosmetic surgery or so, as well as performed alone.

Balloon Synoplasty

FESS  as a revolutionized the treatment of chronic sinusitis and still today’s most important technique is extremely helpful in subjects of safety and duration of the surgery. It can be used alone in patients with sinusitis, which is caused just because of the shortness of the sinus channels, with no nasal polyp formation or it can be used for the patients who require a full sinus surgery. The Balloon Synoplasty method has been successfully used in sinusitis surgery in our clinic. We ask that you make an appointment for more information.

Diamond Rhinoplasty - Dr. Abdülkadir Göksel

Diamond Rhinoplasty

A new way to shape your nose, Diamond Rhinoplasty

Rhinoplasty is the most frequently performed cosmetic plastic surgery in the world among all the procedures. Because of its great effect on the facial expression and beauty, a successful rhinoplasty makes a good contribution in terms of both health and aesthetics.

For years, this surgery was performed by breaking the nasal bones. Because of this, bruising and swelling were seen in the postoperative period. Unwanted and  uncontrolled fractures and wrong healing could be seen due to breaking bones.

Technological developments have begun a new era in the nose surgeries. Thanks to Piezo surgery technique which works with high-frequency vibrations, it is possible to form hard tissues such as bones without breaking them and work on fine details. The diamond tip is used which was developed by Dr. Olivier Gerbault from Paris for shaping the nasal bones. Diamond Rhinoplasty technique has become my routine for rhinoplasty. It is much safer and gives more predictable results .. With this technique, there will be no severe bruising and swelling,  if the patient has no predisposition for bruising and swelling.

Diamond Rhinoplasty technique which was developed by Dr. Gerbault, is a member of International Rhinoplasty Research Society as I am, will change the course of rhinoplasty and spread rapidly throughout the world.

About Life

It is part of my job, meeting so many people, listening to their stories and commiserating as well as sharing their happinesses…If they are unhappy, I try my best to show them how beautiful life is and how we should enjoy every moment. You wouldn’t believe how effective a small surgical touch is. But if you ask me, the key secret in all of these positive results lies simply behind the positive energy you capture and of course mutual understanding.

Rhinoplasty Surgery especially requires dynamic thinking: problem-solving skills, practical experience and creativity. It is truly a tough one. You kind of have to devote your life to this purpose “your surgeries” in order to be good at it. As in all areas of life if you don’t relish it, you can be a good surgeon, but an unhappy person. If you don’t love what you do, there will be no positive energy flow in your environment. I believe that this is exactly my secret. It is from a very classical belief, isn’t is?  I love everything about facial plastic surgery. And I believe, every patient I make happy becomes a new source of energy in me..

“Knowledge grows when shared” is another frequently used statement, but I could hardly chew on. Over the years, while I was getting experience in rhinoplasty surgery, an enthusiastic young doctor team started gathering around me. I wasn’t a teacher, I didn’t have to share my knowledge I gained in years, helping those people was out of my job duties. However, there was another path I could follow. I could guide those doctors and be transparent about everything. So I picked the second path. Only then I was able to understand “Knowledge grows when shared ” and you actually learn more than you teach. A lot of assistant doctor friends of mine did not forget to thank me in their graduation thesis beside their clinical physician. I want to thank all of them here for always keeping me young, dynamic and energetic. Just because they don’t hide knowledge and they share it like me.. We carry big responsibilities on our shoulders “ human health”. Therefore, sharing information has a greater importance to us.

During the surgery, I forget all my daily problems, appointments and programs in my head. I know how much time matters to the patient. It is their most valuable time. I have to respect that and must be fully focused on what I do. Of course in all areas of life “focus” is important but, when it comes to human health- especially the nose being one of the most visible organs and located right in the middle of the face- “focus” has a major importance.

It is my biggest and strongest wish to have joyful, smiling and healthy faces around me. For this reason, I always stay dynamic, try to improve myself with new knowledge, experiences and skills and try to use them for the health of my patients.

Nose and facial cosmetic surgeries are very common surgical procedures which reshape and enhance the beauty of our faces as the most important reflection of our identity. Extremely good results occur in the hands of a well-trained surgeon with a fine artistic view. Aesthetic operations have made lots of people happy.

While I was studying in the Faculty of Medicine, I knew I would improve my self on a creative task. I had noticed my manual skills in early ages. When I decided to be a doctor, I knew from the beginning I was going to be a surgeon. What attracted me the most about a nose major was its ability to combine both an artistic perspective and fine surgical skills.

I want to take this opportunity to express my thanks to my esteemed colleagues for their contributions to my Rhinoplasty journey.
